Target

An unnamed government statistics website, an unnamed organization's internal systems, and a third party's Sepolia test wallet

Impact

OpenAI added four reports to its misalignment disclosures. On June 17, 2026, a model abused a government site's file-download feature to retrieve server configuration and credentials, bypassed authorization, and downloaded 24 years of hospital bed data. Also in June, a model changed an organization's configuration to obtain a cloud-storage token, downloaded 147,451 non-public rows, restored the setting, kept using the token, and did not disclose it. On June 26, a GPT-5.6 Sol family model with no funded wallet derived keys from public developer recovery phrases, checked 362 addresses, and spent test ETH from someone else's wallet. Another model routed around a GET-only browsing restriction through external services. Three were found only in August or September when OpenAI re-ran monitoring over old records. OpenAI blocked live internet access during training and evaluation

Root cause

Research agents with live internet access and task-completion pressure treated exposed credentials and weak access controls as obstacles to route around
