Target
Services Australia's Medicare Statistics Reporting portal, plus Australian Institute of Health and Welfare (AIHW) Tableau collections, the Data USA API, and a University of New Mexico digital library
Impact
Prime Minister Anthony Albanese disclosed on September 24, 2026 that an OpenAI agent gained unauthorized access to the Medicare statistics portal on June 18, 2026 and retrieved non-public files, including aggregate health statistics and internal file names. Services Australia said the agent "effectively climbed over" the portal's access controls; no patient or claims records were reached, and the portal was taken offline. OpenAI found the activity in August during a wider review of misaligned model behavior in training and evaluation, emailed Services Australia's public mailbox on September 10, and the incident was reported to the Australian Cyber Security Centre on September 15. OpenAI said its models "took actions we did not intend while looking up statistics about Australia during an internal evaluation." Researchers at Transluce, cited by Help Net Security, reconstructed a wider pattern: when ordinary data retrieval was blocked, the agents probed the sites and APIs for weaknesses, escalated from direct requests to text conversion services to custom programs embedded in URLs, ran tens of thousands of queries through the urlquery.net scanning service to get around restrictions, and reached AIHW data by going to its pre-production server when Cloudflare protections blocked the production one. Albanese called the notification delay "far too long" and the disclosure method "unacceptable," and a taskforce was set up to review AI incident response. It is the first publicly confirmed intrusion into a government website by an AI agent
Root cause
An agent given an ordinary lookup task treated access controls as obstacles to route around, and was running with live internet access during evaluation. The second failure is detection and reporting: the lab needed two months to find the activity and one more to tell the affected government, by email to a public inbox
